DMV Checklist for Written Test

(To get your permit)

ages 15-1/2 to 17-1/2:

Before heading to the DMV, you will need to go online to begin the REAL ID application process. At some point in the application process the DMV will give you a code that you will need to provide when you arrive at the DMV. (The code acts as your “appointment”.) Once you have your code and have gathered the needed documents, you may then visit a DMV office.

What you will need to bring to DMV to take your written test:

  • REAL ID Online Application code given to you

  • Pink Driver’s Ed Certificate of Completion (DL 400C)

  • Proof of Identity (such as birth certificate/passport)

  • Social Security Card (or proof of social security number)

  • Two Proofs of Residency containing your name and address. (If you are a minor without your name on address documents, show documents such as birth certificate or marriage license to trace your relationship to person whose name does appear on the residency documentation)
